NOTICE OF PRACTICE CLOSURE
Dear Patient:
This letter provides advance notice that I will close my medical practice at Patrick E. Muffley, D.O., LLC d/b/a Embody Wellness Obstetrics & Gynecology at 5957 Cleveland Avenue, Columbus, Ohio 43231. I will discontinue practicing medicine and will stop seeing patients on or about October 30, 2026. After that date, I will no longer be available to provide medical care or prescribe medications through the practice.
This notice is intended to give you time to arrange continued care. The physicians or group practices identified below may be able to accept you as a new patient and assume your care. You are not required to seek services from any of the physicians or group practices identified below.

Your medical records will remain available after the practice closes. To request a copy of your records or have them transferred to another provider, contact embodywellness@hotmail.com. A signed authorization may be required before records can be released.
Please arrange follow-up care and any needed prescription refills before October 30, 2026. For urgent medical needs before that date, contact the practice at 614-600-2979. For a medical emergency, call 911 or go to the nearest emergency department.
